We are happy to help you subscribe your parish to our platform.  Please follow these instructions (PLEASE NOTE: THE PRICES DISPLAYED IN THIS GUIDE ARE FOR DISPLAY PURPOSES ONLY AND DO NOT REFLECT/GUARANTEE ACTUAL SUBSCRIPTION PRICING):

1. Go to and select SIGN UP GROUP




2. Select Annual Subscription and ADD TO CART:



3. Select cart and click on CHECKOUT



4. Select create an account




5. Create Your Account


6. Select the Pay by Check Method

7.  Complete the Payment Information 

**The Authorized By/Purchase Order field is for you to mark the request as needed by your organization for tracking purpose.  A name and/or purchase order number may be used.**


8. Enter Coupon Code and/or Tax Exemption Code as applicable and select APPLY


9.  Continue to Organization

10. Under Organization Information - Select Diocese and then Parish/Group Name - Select Continue to Review


11.  Review your Order and then select Place Order


12. Confirmation of request to pay by check 


13. Account Overview page shows Status of subscription as Pending Active



14. Parish Admin receives email with Invoice for payment


15. Once the check has been received by the Augustine Institute, the check is deposited and the parish admin will receive an email with receipt of purchase

16.  Log into your account with the email you used to setup your Group subscription

17.  Activate Your Organization:

18.  Confirm Custom Registration URL

19.  Go through "Four Easy Steps to Get Started with Your FORMED Subscription"!

20.  To see your Access/Parish Code and Subscription information - select your Initials - SUBSCRIPTION in the drop down:
