Windows 7 & 10: Edge Browser
- While on formed.org click the Menu Button (3 Dots) in the top right corner of the browser.
- Select F12 Developer Tools
- Select the Debugger Tab in the Developer Tools menu
- Select Cookies Tab and select ooyala_storage.html
- Select the cookie titled key_auth0 and right click and select delete cookie
- Close the Developer Tool Menu
- Refresh your FORMED page
