If you do not remember your password to your FORMED user account you can reset it using the "Forgot your password?" link in the Login window. Click Here to learn how to reset your password.
*Important: your password must be at least 6 characters long (it does not need to include numbers or special characters, but we recommend them as a way of creating a stronger password).
If you are unable to reset your password, it is likely due to one of three reasons:
- You did not receive the requested password reset email
- You received an 'error 400' warning when you attempted to submit a new password
- Your password reset link has expired
1) You did not receive the requested password reset email
Once your request a password reset you should receive an email to your inbox within a few minutes. If you do not see this email please check your spam or junk folders in case your inbox automatically filtered this email to one of those folders. You can also search your inbox for our email address, email@example.com.
If you still do not see the password reset email within a 10 minutes please return to the LOGIN window and request another password reset email.

2) You received an 'error code: 400' warning when you attempted to submit a new password
If you received the password reset email and then received the below warning upon trying to enter a new password this likely indicates that your browser does not have Third-Party Cookies enabled which are necessary for this update to be made.
Please update your web browser's settings in order to enable third-party cookies then close out your browser and re-launch. Return to the password reset email and try to enter a new password again. Please refer to your web browser's support resources to learn how to enable third-party cookies.
3) Your password reset email has expired
The password reset request will expire after 72 hours. If you do not open the email and reset your password within that time frame you will need to request a new password reset email in the LOGIN window.
