When you visit formed.org you will use the blue LOGIN button in the top right corner of the homepage to enter the website. Please enter the email address and password you entered during your initial registration for FORMED. Then click the blue submit button.

If you are unable to login, it is likely due to one of these reasons:

  1. An invalid email or password
  2. Using "private browsing mode" or an unsupported local storage (cookie) settings
  3. An unresponsive LOGIN button

1. Invalid Email or Password

If either the email address or password is incorrect you will receive the "Invalid Email or Password" warning. Please double-check that you have correctly entered your email address and password.

Important: If you registered with a Parish Code, this is not your password. A Parish Code is a one time only code that allows you to register as part of your parish subscription. During registration you would have been prompted to establish a personal password of your choice.

If you do not remember your password you can use the "Forgot your password?" link located in the LOGIN window to reset your password.

2. Using "Private Browsing Mode" or Unsupported Local Storage (Cookie) Settings

FORMED uses cookies to enhance your browsing experience and may not work correctly in private browsing mode or with cookies disabled. Some browsers will warn you about this automatically, others will not. Here's an example of a message you might see in  the Safari browser:

Often, an unresponsive login attempt (spinning circle, nothing seems to happen, etc) is the result of private browsing or 3rd party cookies being disabled.

Please turn off private browsing or incognito mode and then pull up FORMED in a new browsing window and attempt to LOGIN. Please refer to your web browser's support resources to learn how to turn off private browsing or incognito mode.

Please ensure that cookies are enabled on your browser. FORMED requires cookies to function properly. To enable cookies, go to this link and follow the instructions based on your browser: http://www.wikihow.com/Enable-Cookies-in-Your-Internet-Web-Browser.

3. An Unresponsive Login Button

If the LOGIN button is unresponsive when you click on it this may indicate that your web browser's settings need to be updated. As mentioned above, please verify that you have cookies enabled in your browser's setting options. Please refer to your web browser's support resources or go to this link and follow the instructions based on your browser: http://www.wikihow.com/Enable-Cookies-in-Your-Internet-Web-Browser.

Once you have updated your browser's settings close the browser and relaunch.
