When you visit formed.org you will use the blue LOGIN button in the top right corner of the homepage to enter the website. Please enter the email address and password you entered during your initial registration for FORMED. Then click the blue submit button.
If you are unable to login, it is likely due to one of these reasons:

- An unresponsive LOGIN button
1. Invalid Email or Password
If either the email address or password is incorrect you will receive the "Invalid Email or Password" warning. Please double-check that you have correctly entered your email address and password.

3. An Unresponsive Login Button
If the LOGIN button is unresponsive when you click on it this may indicate that your web browser's settings need to be updated. As mentioned above, please verify that you have cookies enabled in your browser's setting options. Once you have updated your browser's settings close the browser and relaunch.
